Michael: What did you see in your gallery walk?

  • A lof of the same things.
  • I see a need for common definitions or a glossary.
  • There's not unanimity in understanding what the GMP does.
  • There's strong consensus in our group that the GMP was too inclusive and not concise enough. People who attended to earlier.versions of that stated that things have been added to it.
  • A common language is more than a glossary.
  • We need the GMP to be broad enough to apply to medical school, residency and beyond. This is relatively new and we need to remember that.

Michael: There is a concept that we have to understand that 'done' is done for today. With continual improvement, nothing is ever done. Things can be good enough for now. What we've done is allowed these ideas to crystallize and let them bubble to the surface. People are now feeling gravity and feel like we need to get to the 'real work.' There is a tremendous amount of work that we need to do and all of this is 'real work.'

  • There was a reoccuring theme of a lack of trust.
  • Also of who is going to pay.
  • If you're going to sell this to physicians, you're going to have to also sell it to the practice attorneys. If you don't find a way to protect it, you'll never get the medical community to buy into it.
  • People need to know what has already been done as of right now, relative to competence, education, trusted agent.
  • This is really complicated.
  • We may be reaching a point in this dialogue where it might make more sense to do the work in the individual stakeholder groups than in the whole group.
  • The alliance in this chaordic model is not always going to look like this. It will solve certain issues, but there will still be some things to attend to.

Michael: Because we have all of you in one room having these conversations there is an opportunity to deal with and resolve issues that arise in the dialog. An example of this is a conversation now taking place between the licensing boards and the certifying boards about resolving some interface gaps. I asked several people to draw me a picture of how the medical system works and I think what we're doing now is not as complex as that.

  • Except that we're moving from an already existing system into something new.

Michael: True.

  • How are we going to convince individuals to move into this new system?
  • Anywhere along the line that leads to the physician not practicing medicine, things will stop right there. That's where the battle will be fought.
  • It seems to me that the GMP document has the most footage. That's where we should focus some energy to get more buy-in and refine it further.

In order for you to achieve your ideal final result it is likely that there are contradictions, conflicts, gaps and constraints in the current system that the new envisioned system could resolve.

By statements we mean something like the following:

  • While we want physicians to control access to data about their practices we also want patients and the public to have access to that data.
  • While we want physicians to practice high quality medicine we also want the cost of healthcare to be low.
